Advanced Seating Principles and Manual Wheelchair Selection for Function.
The morning session will focus on the clinical decision making process with regard to custom manual wheelchair configuration selection and the effect on postural support, functional performance, repetitive strain injuries and pain. In addition, proper measurements, wheelchair configuration and propulsion mechanics will be addressed to achieve optimal fit and function of the user. Clinical case scenario’s will be utilized to highlight the concepts outlined above. The Afternoon session will focus on the clinical assessment and application of seating principles to address common problems associated with seating and positioning. These include posterior pelvic tilt with associated sliding out of the seat, kyphosis and forward lean, scoliosis and pelvic obliquity and how to accommodate asymmetrical postures, address abnormal tone, manage primitive reflexes and pressure management issues. The participant will learn to apply problem solving strategies to identify critical product parameters to address these issues. In addition, a comprehensive review of the various types of powered seating available to our clients, including tilt, recline, tilt and recline combinations, seat elevators and powered elevating legrests will be covered. The features, benefits and concerns of each will be presented, including appropriate client criteria and appropriate clinical justifications. Various methods of independent access to powered seat functions will be discussed along with pertinent programming parameters and adjustments. Case examples will be utilized to highlight the principles discussed in this session. Clinical justification and documentation requirements will be discussed.
Course Objectives:
1. Understand impact of axle location and center of gravity and the impact on rolling resistance.
2. Discuss the impact of weight and additional accessories and how they impact performance.
3. Discuss the clinical impact of frame geometries and seating system selection on wheelchair performance.
4. Apply principles of propulsion mechanics and training to the overall user outcome measures.
5. Discuss advantages and disadvantages of add-on power options for manual wheelchairs and their impact on overall function.
6. Discuss components of evaluation process and needs assessment to match client needs to product parameters. Identify critical components and structure of documentation and justification for recommended equipment.
7. Identify what should be assessed in a thorough mat evaluation for seating and positioning and how each element relates to seating selection.
8. Identify at least 4 causes and seating strategies for each of the typical postural abnormalities of posterior pelvic tilt, anterior pelvic tilt, pelvic obliquity, pelvic rotation, kyphosis, lordosis and scoliosis.
9. Discuss how the seat to back angle, hanger angle, footplate angle and footplate position relate to the anatomy of the client.
10. Discuss the differences between planar, contoured and custom molded seating and the clinical decision making for the selection of each.
11. Identify the differences between foam, fluid and gel as they relate to pressure management.
12. Identify the power positioning options, their selection for pressure management, positioning and function, how they impact base selection and configuration and programming options to maximize usage.
